Instructions to use fillable PDF form:

    Click ‘Download PDF’ button – The PDF will open in a new tab.

    Save the PDF to your computer.

    Fill out the PDF on your computer and hit the ‘SUBMIT’ button on the form.

    You may need to connect Adobe Acrobat to your email provider. If connected properly it should open a new draft email with your completed form attached. Send to the pre-filled email address.

    If you cannot connect your email provider: Save the completed PDF form to your computer. Draft a new email to [email protected] and send the form as an attachment.
